In Requiem for a Dream, the Marlon Wayans character's arm gets all brown and rotten from abusing heroin. The arm gets amputated. So perhaps it doesn't quite fall off, but it's close enough.
Two comments. Jared Leto's arm was amputated; Marlon Wayans was his friend. Secondly, in a clinical setting, heroin is an exceptionally effective, stronger, cheaper narcotic than morphine that is used in much of Western Europe, including the UK. It is only outlawed in the US because it was taken up as a cause by a temperance fighter in the early 20th c. Heroin is a 2. Heroin's illegality and illicit abuse is a 1.

You're right, it was Jared Leto; Wayans ends up in jail.
If my definition was partitioned that way, those ratings might well be correct. But it is not.
